Was their yellow book accepted as proof of address.

 

Also can you get a medical certificate from any clinic or hospital or does it have to be one on approved list

I did that. The yellow book wasn't accepted. They require proof of address from the immigration, which takes seven days. A medical certificate from any clinic/hospital is sufficient. You also need an appointment at DLT Nonthaburi. You can schedule this online, as well as the one-hour safety film.
