Dont understand why you need the photo, now in at least Bangkok they take the photo and insert it on the new license, looks just like a credit card.

Copy of passport, main page, plus visa page, plus TMwhatever page..last entry into Thailand.

Doctor letter saying no disease and fit..100Baht

Letter confirming address, I got free from immigration

Completed form

Brought photos but no good as they now issue credit card type licence with digital photo.
