Could someone please post the link to the latest form, as the 2004 link posted as a sticky does not seem to work.

Thanks UJ...when I click on that link a blank page comes up.....it looks like a separate link is currently under construction on their website....are there any other links where the form can be found?

It opens fine for me. Some browsers have trouble with certain PDF documents. I suggest you try downloading it and then trying to open it.

@tazly just for my peace of mind, what nationality passport do you travel on?
