Passes are issued by the ministry dependent on travel dates so if someone has applied weeks in advance they'll have a long delay as others travelling sooner will be prioritised.

They're given out on the incoming flight as the arrival bit is retained by immigration and the departure bit remains in your passport until you leave. If you lose the departure bit another form can be obtained at BKK.
