Correct. The OP mentions "hotels for one day's quarantine" so the plural indicates 2 days but the "one day" made me unsure so thought it better to point it out. Not sure where the turnaround time thought came from. -

Passes are issued by the ministry dependent on travel dates so if someone has applied weeks in advance they'll have a long delay as others travelling sooner will be prioritised. -
He wants 2 x one day bookings not 1 x 5 day booking which is where your link goes. They're available on that site on the next boxes along, but the OP may not notice them.

Should be two days for Test and Go. -
You can if you have booked from your origin through to Phuket on one ticket, but you can not buy a separate ticket from Bangkok to Phuket to add to an existing one from your origin to Bangkok.

Only 12 digits. Insert another zero. You had the right amount of zeros in the OP even if you said 68975 had 6 digits when you meant 5.

It's a commercial site and offers Test and Go in several locations. Probably just an old message that hasn't been updated as things have developed. -

They're given out on the incoming flight as the arrival bit is retained by immigration and the departure bit remains in your passport until you leave. If you lose the departure bit another form can be obtained at BKK.
