Travel agencies has a combo ticket with Seatran ferry and Phantip busses for 550 baht. 

 

They will pick you up with minibus, take you to ferry and bus to Surat Airport. 

Ferry and bus around 4h, minibus depending on your location. 

 

They will plan your trip for the flight.

Also if you want to use Raja Ferry (if it's more convenient), they do a joint (combo) ticket in the ferry booking office.

Online it says 400 for the ferry and bus to the airport. 1.5 hours for the ferry, 1.5 hours on the bus, and a short wait at donsak for the bus. I don't see any difference in price if you want a hotel pick up, so you might want to check it out?

Lomprayah is the fastest ferry. Like the other ferries they sell airport transport tickets. Always a specified bus with no other stops.
