A completed TM8 form with a photo attached. Fillable TM8 re-entry permit form.pdf

Copies of your passport photo page, visa, extension stamp, arrival stamp and TM6 departure card. And 1000 baht for the fee.

thanks a lot

Keep in mind you can obtain the re-entry permit at the airport as well as long as you have a valid boarding pass.  Same price: Single B1000, Multiple B3900.  Usually takes much less time than at Immigration.

At BKK Passport control, usually takes 15 minutes.  No form or copies needed.  They automatically print everything out.  They  insist on taking the photo: B200 additional.

At DMK, Re-Entry shares the office with "Overstay", just past security.  Make sure to let them know you are there for a re-entry when you enter or they might assume you are there for overstay which can result in a much longer wait.  I don't recall if they have the same automatic application system as BKK.

Remember that at Don Mueang, the REP desk is AFTER you get stamped out of the country at passport control, but at Swampy, it is BEFORE you get stamped out. I usually get my multi-REP at DMK, so once when I had to get it at Swampy, I went through passport control and started looking for the REP desk. Turned out I had to go to the IO desk, get my stamp voided, go back through passport control to the airline counter area, find the REP desk, and get stamped out again. There was a problem at the IO desk that took a long time for the IOs to resolve among themselves. There was also a long wait for the REP and my plane was already boarding. Told one of the staff and they said for me to get in line at passport control. It was a long line but finally it was my turn to go to the passport officer. Since I didn't have my passport, I turned around and was going to let the person behind me go ahead, but the IO got mad and started yelling at me, telling me that I couldn't do that. So here I am standing in front of him and he's asking for my papers. He was fuming, then I pointed to the REP office. Fortunately, the IO lady came out and brought my passport to the passport IO. He was really confused. Probably never happened to him before.

Fortunately the plane taking me on my way to the US was waiting for me, the last passenger.

Unless things have changed, at DMK, the REP desk will do everything for you, even copying the needed pages in your passport. They didn't charge anything extra, but once when I handed over B4,000, I didn't get any change back. When I asked about the B200, they laughed and said "Tip, tip." Reminded me of a bar. But I did get the B200 back.

The first time I went to the office, I went to the wrong desk (on the left) and was waiting for a IO to call me. After no one did, I went to another desk on the right and found out it was the REP desk. So it's right that you have to find and approach the right desk.

Your making obtaining a reentry permit at airport a difficult process. The opposite is the case. You didn't even have the TM8 form with you and photo etc.

The office at DMK has clearly makes sign above door "Reentry permit".

Nothing could be more simple.

You didn't get the 200 back as they photocopied pages and pic etc and did the TM8. Same "service charge" is applied at swampy. Did it yourself and the charge is 3800.
