Appointments are not mandatory but they do save a lot of time.

OP when you make your booking you will receive a confirmation email.

Make sure that you print out hard copy. 

There is photo/print stall lower level where banks are and they can print quickly.

Also don't muck around with the main ticket desk. Walk straight to L 32 (?)

They will not call your name. When io is available approach and hold your appointment print out. Be 15 mins early as they often see you early. 

Process takes about 40mins

I have seen some sheets of requriements say you need the Thai health insurance and some do not. Do you know want they are now working with ?

Medical insurance is not required unless you are extending a entry from a OA visa. If you did not need it last year it will not be required now.

People from other provinces can't have their extension at Chaeng Wattana ? only people who live in Bangkok ?

The extension has to be applied for at the designated office for where you are living.
