Form for travel out of Bangkok

I have been lucky to get an appointment in Bangkok on Thursday for AZ vaccination.

I understand I need a special form for me to return to Buri Ram the following day.

Has anyone a copy of the form I need to complete please ?

I was playing with this form - accepting thai ID format (not passport, which in my case has also letters, not only digits). But after putting all digits from my passport, I topped up with random numbers 012345.

Application has problem with selecting proper areas with entered postal codes. But looks like good enough

Why go back to the UK? There are far better places to go. If they were advised, they would have been aware of the UK non-resident tax rules. Any other country, or multiple countries, rather than the UK would have been better. If they kept a "home" in the UK then they likely weren't fully tax exempt anyway. It will be interesting to see the long term future of Dubai under the situation. The rulers must be accutely aware of this.
