The narcissism has always been there. That's why we've seen him react with what is termed narcissistic rage when challenged or thwarted in his ambitions. Don't know about the dementia, as Trump has never maintained any consistency other than manipulating government and finance to make himself richer at the expense of regular people. And, btw, look at what he's done to the dollar today. Now down to 31.60 baht to the dollar.
