I would like to know where in KK or Chumpae to get a medical certificate for a DL. I am sure I can go to the private hospitals, but don't want to pay an exorbitant fee. I know in BKK there are places to get one for 200-300 baht. Any places you guys have dealt with? Thanks. 

go to a private clinic there are no doubt many in your area,

private hospital 

(absolutely last option)

or you could go to a public hospital but I wouldn't recommend that either  unless you like wasting a whole day.

The local public hospital is the cheapest option. The trick to conserving time is to get a nurse at the nurses station to complete the certificate and by-pass the queue to see the doctor, maybe took an hour and a half total.  I think I paid 50 baht.

In Chumphae clinic at the corner with HW 12 opposite police station. 150 or so. No checks at all some years ago 😁

The other one I have used is at the road at  very end of the small bus station.

He at least checked blood pressure and I had to confirm that I am not epileptic or lunatic😁
