Where Do I Get A Tm 47 Form?

Can I get this form online or is there some place I can pick one up besides the immigration office?

I need to report by August 8. When is the latest date I can go to the post office?

Any help would be appreciated.

Mail 7 days before due date.

Notification by registered mail

Copy of all passport pages (up to the latest arrival stamp in the Kingdom or latest visa stamp)

Copy of arrival/departure card TM. 6 (front and back)

Previous notifications of staying over 90 days (if any)

Completely filled in and signed notification form TM. 47

Envelope with 10 Baht stamp affixed and return address of foreigner for the officer in charge to send back the lower part of form TM. 47 after having received the notification. This part must be kept for reference and for future notifications of staying over 90 days.

The above mentioned documents must be sent by registered mail and the receipt of the registration kept by the foreigner.

Mailed it on July 14 but have not gotten anything back in the mail yet. Approximately how long does it take for them to mail back the lower part of the TM 47?

Might well take 3-4 weeks as Immigration processes them in their spare time.  Do hope you kept the Thai Post EMS or Registered receipt as sort of proof of mailing.  I do mine by EMS as can track online.
