Retired. - Job status N/A, Manager N/A.

For extensions based on Thai family/spouse, they have also introduced another new form STM10, which requires a witness's signature.
'Affidavit of Family Relationship to Thai National'.

Apart from the usual TM7, STM2, and STM9, the new form STM11 applies to all extension applications, whilst the new STM10 form applies purely to extensions based on Thai family/spouse.

You can download and pre complete these forms here: https://bangkok.immigration.go.th/en/downloads_en/

For Name Of Company/Partnership - I was told to just put my name and address.

For the line below Coordinator name - I was told to put my name again!

Last month at CW for retirement!

Applied to extend Support Thai wife visa this week at Samut Prakarn office.

Wife and i filled all of the forms you mentioned.

No witness signature required for STM10 (could be because we're regulars for past +20 years).

I was also given 2 forms only in Thai and told to sign, asked the wife what they were, she reckoned I was giving my consent to have my details checked (strange, as I did a form in English for that).

Some reports that the IO's have signed as a witness, not so fortunate in our case.
Next year we'll take a witness along to avoid a revisit.

And your Immigration office is ????? Why do members continue to ask immigration questions without naming their province, STOP WASTING OUR TIME !

I was at my local IO yesterday for a 90 day report, I met a guy I know in there who was doing his annual extension,  he mentioned about the new "witness / affidavit requirement"

When I was collecting my passport from the counter the IO  told me (without me asking), "when you come next time for extension, you must bring a signed copy of somebody's ID for witness"   wife asked if her brothers ID was acceptable and the IO said "yes anybody's is ok"

I asked if they needed to complete a form or attend the office in person ( as that's what the STM 10 form implies)  and she said "no need"   just bring signed ID !   Make of that what you will

At Trat Laem Ngop, we have to take our witness and they have to be present at home visit, every year ! we live on Koh Chang, so not very convenient, plus I have to pay our witness for adays wage and her ferry fare both ways.

Note I have put the name of my Immigration office !

Why thumbs down to an obvious question ?

Ignore it.

Stating the immigration office should be made clear in every thread.

Op requested information about completing a new form that is being issued at all Immigration offices across the Country.

In this instance, the office in question is irrelevant.

How do you know that it is country wide  and not limited to particular offices  if the poster does not mention their office location ?

It's not new  Trat have ALWAYS wanted a witness statement.

Op posted about the new STM11 form, which doesn't require a witness.

Posted in other threads, and links to the new forms for download.

As I said nothing new my immigration office gives me that in Thai. They fill it out I just sign it, like all the others, My immigration office just ask me to come back in 2 hours, we go and have lunch, and all I do is sign them all. 👍to Trat immigration.

STM10 and STM11 are new this year.
They are in both Thai and English.

Some questions are generic, meaning the same answer applies regardless of the Immigration office and naming the Immigration office is irrelevant in these cases, such as this thread.

Naming the immigration office at all times IS HIGHLY relevant IMHO. In this particular instance, whilst the STM forms downloadable from  https://bangkok.immigration.go.th/en/downloads_en/ may well be OK for CW, it doesn't follow that they are for other offices. For instance, the versions of the STM2 and STM9 which Rayong handed to me for completion a couple of months ago in connection with my latest retirement extension application are in no way 100% identical to the versions downloadable from https://bangkok.immigration.go.th/en/downloads_en/.

Individual offices tend to be ultra-fussy about which versions of these forms they require you to complete.

How much do Immigration charge for an extension?

STM11 requests a witness signature, an STM10 does not.
Previously reported sometimes the IO signs as the witness.

What a pathetically sarcastic attempt at saving face on your part. Welcome to my Ignore list.
