August 1, 20169 yr There have been some posted but unless lucky the one you download might not be the correct form for where you are applying. They differ from office to office.

August 1, 20169 yr I just did today buy a 30 Day extension. I had to fill out the mentioned form. I left it blank. No problem. But there was also two other Forms. One for the landlord which i filled out by my self. And now, 2 Passphotos and 2 sets of photocopies of Passport needed.A lot of paperwork.

August 2, 20169 yr Just beware, though, each immigration office appears to have its own version of this particular form. I had completed an electronic version which I had downloaded from here in preparation for my latest retirement extension application at Maptaput 3 weeks ago, only for this to be rejected on the grounds that the photo which I had affixed in accordance with the instructions given on that particular version was not, in fact, needed! I was handed a scruffy version which I had to complete in manuscript on the spot instead.

August 2, 20169 yr Filled in 2 of these forms in 5 days. Last Thursday at Suksawat for my 90 days and today again for extension of Non O at CW. I wonder gow many times they will ask me to fill this in.

Also tried to upload the word version without success
