Sign the car's blue book now, she can add her info later.

You might not want to carry the signed book in the vehicle if you do that....

No, she could not do that, legally, with a car that does not belong to her.  The car would become part of your estate and then inheritance laws/your will, would apply.

Can I can transfer the car to her name at the Land Transportation Office,if so what documents would I need ?

The car's blue book or the motorbike's green book should never be carried in the vehicle unless it's being sold that day. A photocopy works fine if asked by the police.

No.   

A will would work, or POA easier, but sign 2, leaving one blank (but do reference car in blank one).

 

Signing blue book might work.
