TM8 for a re-entry permit.

Apply at immigration or if flying then can do it on departure at DMK or BKK.

Immigration gave me mine with 

My 12 month non 0 as I told them when I got it I had to leave for 3 weeks

Seems a 12 month extension.

No problem. 

As mentioned you can obtain reentry permit at your immigration office or airport.

1000b single reentry and 

3800b multiple reentry.

At the airport they have optional "service fee" 200b to do the form and provide photo.

Yes you simply obtained reentry permit at same time as extension.

Many folk do that. 

Thanks was not sure what i was doing when I got my 12 month extension but have to go do my 90 day so thought I would do it then.
