My Thai girlfriend neglected her teeth before meeting me. In the west she would have had a brace from her teenage years, which her younger sister has had, but she went without and as a result her teeth are angled forwards a lot. Not unsightly and quite natural as she has a naturally square shape jaw, but still something that could be fixed.

She is 32 now and wanted a brace but the dentist said two upper back teeth would have to be removed which she needs to go to hospital to have done as last time she had a tooth removed there was excessive bleeding.

The excessive bleeding, according to a recent blood test, could be down to mild thalassemia. The doctors actually said she needs a second 'big blood test' for this. What exactly does this mean? What is the 'big' test as oppose to the one she just had?

Thirdly, her teeth are very yellowed even though she has never smoked, doesn't drink tea or coffee, and is no longer a regular drinker. (She was before for a few years)

The teeth actually look weak and almost transparent and two of the upper canines are mild brown. One upper tooth needs a filling.

Her breath has improved with using Corsodyl but it still seems bad.

I'm not sure what to do next and where to go to in Thailand. The dentist wont do anything.

Alpha thalasemmia is very common among Asians. My wife is Singaporean and has it. In fact her whole family have it and all of them are very active and long lived her grandmother making 102 and her mother 98.

All of her family are very active and she just keeps an eye on her annual blood tests to ensure she is not anemic.

All your GF needs is a standard complete blood count (CBC), one small vial of blood and results within a week. Shouldn't cost more than a few hundred baht.

Read this link.

Absolutely nothing to worry about.

Re the teeth, standard for Thai women, it's all of that sugar cane they eat (my Thai "sisters" husbands hates me as I always bring them over large sticks of rock to which they are addicted), but any good dentist will do a clean up and offer advice as to what is required. Where are you located?

She should go to the hospital where she is registered under the government health scheme. With a history of excessive bleeding, she does indeed need to have the extraction done in hospital, and this would also be the place for thalessemia work up.

If the hospital is unable to do what is required they will refer her to a higher level facility.

Thanks, that's helpful to know. We're in Bangkok so there's plenty to choose from. The previous test was a complete blood count.
