I have an appointment in Bangkok at the "Division of Communicable Disease Office" to collect my Covid Vaccine passport. But I have no idea where it is. I tried looking on Google with not much success. I have attached a picture of the booking appointment.
Any advice would be great.

They sure don't make it simple.  Might want to email them, ask for Gmap location.

Apply as early as possible, as they state process times are "long" due to the number of people requesting the vaccine passport.

Pity the post option does not work on the application site, then you would not have to go there at all! As you have the vaccination certificate the book is just a duplication.

For Grab - Pasting the Thai - brings up a dozen locations.

The English name - Bureau of General Communicable Diseases brings up the right spot (I think).   Good luck.
