My recommendation - follow Sheryl's advice, closely.

I have had prostrate issues for about 6-7 years.  I have been seeing two very good urologists at my local government hospital, both of who provided similar advice as Sheryl.

 Hi Jim..

What medicines are you taking?

and do you have any side effects?

Mine are not too good and am looking for some way to stop taking my dutasteride and 400 micrograms Tamsulosin daily..[ all from Uk]

This is a very important issue @p414. I reached your stage after being on meds for a few years. I live in the sticks and all the local urologist could offer was surgical removal. No way Jose! I was aware of the risk of kidney damage as described above by @BuddyPish and decided to chance my arm at self catherization in order to relieve the pressure on the bladder and kidneys.

That was over a year ago and it's been so successful that I 've continued using this method. I'm no longer 'toilet hunting' whenever we go out anywhere as I can last for up to 4 hours between pees and importantly I only need to visit the loo once during the night so my sleep has greatly improved. Like you I practice intermittent fasting, nothing after 6pm which also helps and I've also come off the meds which were, by now ineffective and giving me nasty side affects, notably weight gain and edema.

There's a very interesting website right here if you'd like more details and you can PM me if you'd like to discuss further. You should of course also chat with Sheryl about it.

Thanks Moonlover..

I should have said that a couple of times a week I urinate only about an eggcup full..Flow is ok.

A US based (youngish) Urologist has prescribed me 2.5mg Cialis twice a day.  Yes, I was skeptacle as well, as Cialis is designed for ED but after taking the big three tamosolin, artrovat, finesteride, for a few years..........went to him and he prescribed 2.5 Cialis twice a day,,,,,,,,,,,after 3 or 4 months now, I no longer get up in middle of the night, last exam prostrate has shrunk, and bladder is emptying normally.  He killed two birds with one stone...........Now he is considering a Water Vapor Therapy called Rezum, I believe it was developed in Germany.

@Sheryl What are the options if one has Thai social security?

Whatever is offered at your nominated hospital. In most cases that will be only TURP.

I just wanna know what types of treatment are covered by Thai social insurance

There is no restriction for treatment (except probably not gover implsnts such ss used in Utolift) but you are restricted to your registered hodpital snd highly unlikrly this offers othrr thsn conventional TURP.

I am most interested in the 'Urolift' treatment.. This sounds much better to me than surgery.

Urolift does not last very long.  So unless your life expectancy is only a few years, this is unlikely to be a long-term solution

This is interesting! I am currently on  Duodart which I dislike. I am going to ask my urologist about replacing Duodart with Cialis. The cost of Duodart may be comparable to Cialis. Unfortunately, to the best of my knowledge, generic tadalafil pills are not available in Thailand.

Be  carefull,of Adovarte DUSTERIDE side effects. It stopped my husband sleeping for 2 years, he became so weak and hardly closed his eyes. I researched Adovart Dusteride and found many Forums of people experiencing the exact same problems. The first day he got off this drug he slept 14 hours straight. It took  6 months to clear this drug out of his system. His PSA is now 2 and he takes 1 Havasu Saw Palmetto a day.

Thanks lacessit..

Where is 'gemini?..I want something that means I do not have to keep taking the medicine I am now taking daily.

Gemini is an artificial intelligence program which can be asked medical questions. Access it by typing Gemini into the Google search bar.

Once in, you can type in a question such as "What are the risks of Urolift surgery" and it will answer, usually less than 10 seconds to respond.

It won't get involved in political questions, which is probably not such a bad thing.

Thanks for that.

I have problems with my medicines...I have gone years without even taking an aspirin until I got BPH.

What places stock this herbal treatment?

It is intended to be a permanent solution. Follow up data at this point is for only 5 years as the device is new. 5 year results are favorable.

Should note that this, like Rezum, is suitable only for relatively small prostate size.

Laser treatments can be used with larger prostates

If you have no problems and your prostate has shrunk why let your urologist consider rezum?

Don't laugh but a handful of Sunflower or pumpkin seeds every day works wonders ! (and no, I am not a veggie or Vegan)

I am two years in to the Tamsulosin and Finasteride  regimen which generally seems to work as intended. But have also recently noted some related advocacy for pumpkin seeds and tomato extract. Am now interested in an attempt to clarify issues about potential contraindications.

I hope it is better for me than the 2+ years of side effects I have been getting from taking Dutasteride [brought from UK.]

I have decided against surgery .

As an aside, anyone taking regular medicine ...Check out 'Medisafe'...they are on ,'Line'..

Thank you for this, Sheryl.  I was advised that the key 5 year underlying evidence is from a manufacturer affiliated study putting a question mark over its reliability.  Moroeover, even this is just a single study and only 5 years.  So I was advised that if one views this over a longer period, like 10+ years, one will most likely need a conventional operation on top of this as the prostate just continutes to grow over the Urolift implants with a later operation at older age being much more dangerous.  The "EAU Guidelines on Non-Neurogenic Male Lower Urinary Tract Symptoms (LUTS)" in their current, i.e. 2024, edition concluded as follows in view of the totality of the evidence currently available:
