So have wife sign it and go with the flow.

There were more forms to fill in this year at Jomtien I'm pretty sure that this was one of them  even the immigration officer commented that there are more forms   this was after the rain forest in duplicate was submitted and the fee paid....at this point quite frankly I was in autopilot and just wanted to get the hell out of there.

My wife was first presented with and asked to complete that affidavit in 2022, and again in 2023.

Last year it was presented in a new format from a template on their PC, with all the details included, just required signing.

Chaengwattana has been using that form for quite a few years. They hand it out to you at the document check stage (Or at the officers desk) you just write your name on it and sign it
It's in the packet of forms that have the overstay acknowledgement, the terms and conditions to your extension, and that "statement" (the STM10

Probably. In addition to the stack of paper I bring to those "extension" appointment, but IO generally add a few other documents to the even larger stack of documents that are plopped in front of me to sign.  Dang, I need an auto-pen like Joe Biden.

In answer to the OP, my assumption:

 

Top section, Thai wife's details?

 

Item 1; Husband's details?

Item 2; Husband's details?

Item 3; Husband's name?

Item 4; Wife's occupation and signature.

 

Please correct me if I'm wrong, thanks.

Wife has been filling in and signing one of those for several years.   It's usually stuck in front of her while other papers are being checked by the Officer; nothing new in Chiang Mai.

I had something very similar on my recent retirement extension.  I could not work out what it was about as it seemed totally irrelevant to my  extension.   IO just sign sign it, which I did.  It does make you wonder what/who is creating these things and for what purpose.

When I had arthritis 2 years ago I could hardly hold a pen to sign all the paperwork. The immigration officer recommended getting a stamp made in my name for the printed part which cut the writing in half and speeded things up a lot. Now wife stamps my name and passes each paper for me to add my squiggle. Stamp cost 200 to have made locally.

Same here in Saraburi. Changes to paperwork every year but same result. Just waiting for mine to return from Bangkok

Yes.

I submitted the paperwork for my 1 year marriage extension yesterday (Songkhla Immigration - Hat Yai).

 

I have digital copies of the forms that I update each year, this year 3 documents were returned and 2 replacement forms given to me and my wife for completion.

  • The 2 forms the wife previously completed 'wife document 3' and the 'certificate from host'  are replaced with a single document the OP's 'STM.10 Affidavit of Family Relationship to Thai National' .
  • Additionally, previously 'document 5 - consent to disclosure of information'  is now 'STM.11Consent form for Fact Checking'

Helpful advice however in reality folk can attend immigration and sign provided forms "on the spot" 

Big deal over nothing..

The wife is in attendance. What's the issue.

Did I say it was an issue?

I just prefer to download and complete ahead of attending the appointment, it saves time.

It is common in  China and South Korea to have a personal name stamp that is used on documents.

Many folk do not have home printer etc.

Waste of time imo.

The forms for acknowledgement of overstay basically just require a signature.

The other slightly more.

You can fill in while io does doc check.

Thread is making something out of nothing.

I wouldn't dispute that, indeed I haven't, I'm just one of those annoyingly organised people who like to prepare everything in advance, and everything is in duplicate in the provinces.
