Application form to gain a COE, Thai PR holders

I've tried to find the application form and the list of documents which must be submitted but I can't find it.

 

Would very much appreciate a cut and paste or a link etc.

For full details contact your local Thai Consulate or Embassy. Also check out your Thai Consulate or Embassy Facebook pages. Although similar some countries may have different requirements.

Then your option,I guess, will be the consulate in Sydney.
