Remember this is about Thailand, couldn't care less about another country

Over the years I have acquired quite a good insight into the Thai medical system. This regards both large and private or less big hospitals both in the cities or in more remote areas. One of the big differences is that the business model of a hospital regarding income is not based on a fee for the doctor but on testing and selling drugs. I have also travelled a lot and seen hospitals and doctors work in other countries. The approach to counselling a patient and choosing for the optimal therapy is in by far the most cases based on genuine belief that it is best for the patient and I do not think that ethical standards for Thai doctors are very much different of those elsewhere. But it is true that in Thailand there is much emphasis on doing testing, which is often not really wrong but also often not really necessary. I have advised a number of patients here and done some second opinions where it comes into my area of expertise and can only say that the quality of treatments advised and performed are very standard. In one special case of a very complicated condition it was actually world-class treatment.

OP. what medication did your UK doctor put you on?

I have BPH , classed as obstructive although I pee normally. Currently on a combination of dutasteride and tamsulosin. Previously, I was prescribed finasteride and doxazosin.

I asked my urologist if I would be better off having a TURP or similar surgical procedure. His advice was to stick with medication for as long as possible, because surgery carried a significant risk of creating other problems, such as impotence and retrograde ejaculation.

And in some cases incontinence.

I went the Rezum steam route at Bamrungrad Hospital.  Below are a few of the previous related topics which might be worth reading.

Forget the surgery, it's a last resort. I have been symptom free for the past 3 years after smoking 0.5gm of high THC Indica cannabis every evening just before bed. No other medications required.

just because it works for you doesn't mean to say that it will work for everybody, I just did a simple internet search and came up with this link.

https://www.healthline.com/health/sativa-vs-indica#cannabis-strain-chart

There are at least 15 different strains of cannabis and sativa with CBD contents ranging from 0,1% to 6% and and a THC ranging from 13 to 30%. They also vary in the conditions that they can treat.

From the link

Remember: The potency of cannabinoids and terpenes vary among growers, and while certain strains may be helpful for certain conditions, your own experience may vary.

There is much more information in the link if anyone is interested.

Thank you for making me do a search, as I am looking to find something to relieve osteoarthritis in both knees and a persistent back pain.

I had not realised that there were so many different variations.

I have chronic osteoarthritis of the lower spine, and pain from some kind of sporadic quad strain.

While everyone is different, I can play 9 holes of golf ( walking ) pain free after drinking half a cup of kratom extract on an empty stomach. It may help you.

Caveat: It is addictive.

Most of the things I have learnt about cannabis and similar came from the cannabis forum. I saw the doctor at the hospital at Kamphaeng Phet yesterday and asked him if CBD oil would help.

He was not sure and asked me to come back in a week and he will have talked to colleagues and read up about it.

I find that the Health forum is very helpful though there are many keyboard doctors an surgeons here.

As I said, everyone is different. CBD oil does absolutely nothing for me, and I have taken it well above the recommended dose. THC acts as a sedative, and affects my balance. I have never got to the euphoric level pot smokers or eaters report.

I buy kratom infusion locally. It's also available on Lazada, very inexpensive. The last time I looked, 49 baht for 1 kilo of leaves.

The infusion formula is ten leaves boiled with 600 mL of water. Quite bitter in taste, I usually have a slice of mango after.

Your bph will go away if you clean up your diet, exercise, get to your ideal weight and believe you can do it.

A really important component is your thoughts.  As in the placebo affect.  But without the medicine.  Have you considered how important it is to give the appropriate commands to your brain so the body can execute them?

Resulting in a fully functioning healthy body.

There are so many wildly different strategies posted here but the degree of belief in that strategy seems to be important to getting the results desired.

No, it is not.

Conventional surgery is a resection not removal.   They just trim away excess tissue which is blocking the urethra https://www.mayoclinic.org/tests-procedures/turp/about/pac-20384880

Alternative treatments also reduce some tissue to shrink size, through various methods.

Brachytherapy and other forms of radiation are nto used for BPH

I thank you for your information regarding surgery.

However, I can assure you that when I had HDR Brachtherapy at St James hospital in Leeds to clear up some residual malignancy, 2 men on my ward were waiting for treatment for BPH.  From what they told me, they were waiting for the same treatment that I had just had. As far as I know, and I could be wrong - I'd just come around from my anaesthetic, the department I was treated in, only carries out Brachytherapy.

However, and I have no wish whatsoever to get into an argument about this but this study shows that it was being trialled in 1994 - with it appears, quite some success.

To quickly reduce BPH try 1/2 desertspoon of turmeric plus 1/4 teaspoon ground black pepper in 1/2 glass water. Daily.  Incredible systemic anti inflammatory.

Combination is synergistic, multiplying the Turmeric efficacy, and ejaculate as often as applicable.

Do get checked it is not a cancerous prostate.

Be wary of mechanical and drug interventions. Finger massage? dunno. Maybe not needed.

Even urologist's digital exploration has limited access to the complete circumference of the gland.     Anal ultrasound preferred option?

You didn't mention your PSA rating, which is only a partial indication anyway.

If cancer or not, take the harmless anti inflammatory.

How do I know? Been there, done that, 30 years now, and living a comfortable life with a flow like a horse.    Well --- a small horse.

Dirk Z   This is not amateur expert opinion. My Urologist said 30 years ago to stop his medication and keep doing what I was doing.

Scubascuba3 is on track. Many medical specialists know limited options.

30 years ago.... seems not state of the art...

You can do it yourself.  I choose to have a female massage my Prostate.  Just make sure they don't have long/sharp nails.  That and frequent ejaculation, healthy diet, and not sitting much relieves it greatly for me.

Frequent ejaculation didn't save me from prostate cancer and my job required me to not sit much, so it must have been the diet.

Alas, I am so extremely cynical that no placebo would ever work on me.

He's right!

After I had my TURP operation I have had nothing but trouble.....so much pain, I was taking Oxy ....that's another story...Xylophone is the only guy that knows my story.....but 4 years since the operation to reduce my prostrate size, I am impotent  have to wear a diaper all the time because of leakage.....tried yet another op and nearly died from Septasemia,  heart failure , renal failure...and this was caused by one of the top hospitals in BKK

I should have had rezum , a lot more expensive, but in  hindsight I really wish I had......I have spent almost 2m in hospitals here, but things have calmed down, no pain now and I am off Morphine and Methadone.

I get the odd UTI now and then but have Monurol for that, but urologists in Thailand's supposedly best hospitals really made a mess of me.

Don't have a turp....putting metal inside such a delicate organ is just not a good idea.

Good luck

I remember you had a thread on the travails you were going through. My Australian urologist said Rezum was no better in terms of side effects.

I only have to look at the relative qualifications of Thai urologists compared to my guy in Australia. He has the suffixes FRCS, FRACS after his name, and those don't come out of cereal boxes.

I'll trust doctors here for routine stuff, anything more complicated I am on a plane to Australia.

Comparing doctors in upcountry locations with, for example, the best available in Bangkok (where there are urologists with the suffixes you mention and US equivalent) is comparing apples and oranges.

That said, the range of treatments for BPH available here is more limited than in the West with several types still not available and others (e.g. Rezum) available in only a few places and likely still on a learning curve.

Fair enough, although as you know my recent treatment regime for something else was simply unobtainable in Thailand. Even if it was, megabucks.

Another gripe I have is the pathology result which put me on a plane to Australia took three weeks. In Oz, a couple of days.

Personally, I don't consider Bangkok is liveable for more than one day.

I had the TURP operation for an enlarged Prostate in Bangkok a couple of years go with no side effects so far. Only minor inconvenience for a few  days but no urination issues and no midnight trips to the bathroom since. I was 80 then and had been forced to use a catheter a couple of times when unable to urinate. Everyone is different and only a good urologist can advise.

Sorry to hear that, but you were unlucky. Millions of men have had TURP and not have had a problem. There is ALWAYS a chance that ANY operation can go wrong. I was also unlucky with my prostate surgery, to my everlasting regret.
