I think it only applies to ladies but in some places in Bangkok no problem. 

I never wear underwear when going for a foot massage, usually it escalates

I only wear underwear (swimsuit) when i visit a doctor ...

My GF used to have that fro. I asked her to trim it but she wanted to look "natural" and kept it "untouched". Had no choice but to dump her. That was kind a thick fro.

I prefer to wear underwear because it feels more comfortable that way. My nuts feel sweatier when I don't wear them. They probably aren't sweatier but they stick to the inside of my leg and it it feels sweatier. But do as you please. I haven't been approached by an underwear checker for a number of years now so you might be as lucky as me. But be careful when you wear shorts. Sitting in public underwear-free with your legs open might cause some backlash. I mean, you wouldn't want to be the focus of attention of irate Thai netizens in a social network posting frenzy.
