The statistics say unprotected anal sex is considerably riskier in terms of HIV transmission than unprotected vaginal sex, irrespective of whether that anal sex occurs with a man or a woman. A vagina has natural lubrication, a rectum/anus does not.

I ran into a post op once, it kind of grossed me out. 

 

It's useless as a usable sex organ and basically a nasty looking version of the real thing cosmetically.

 

I think it amounts to irreversible mutilation, it goes way beyond the territory of letting people be who they want to be sexually. 

 

I think those who do it have not been properly educated as to the gravity of the process.

Hormone therapy or not, transgenders still get their sex drive from their testes, and when they lop those off, it's game over for sexual desire. I don't think that's very wise.

 

I think anyone, male or female, not actually deformed, who go in for facial surgery  are equally at risk of dealing with irreversible changes they may hate so much they end their lives.
