New Immigration Form STM.10 Affidavit of Family Relationship to Thai National

Anyone else had to fill one of these in for an extension of stay, marriage?

Unsure of whose details go where as it's another one of those dreadfully put together Immigration Forms.

From random internet search.... 

"fyi - Just finished marriage visa extension at Div 1 Immigration in Bangkok - Chaeng Wattana. They requested a new form STM 10 affidavit signed by Thai wife in addition to everything else." 

 

So have wife sign it and go with the flow.

There were more forms to fill in this year at Jomtien I'm pretty sure that this was one of them  even the immigration officer commented that there are more forms   this was after the rain forest in duplicate was submitted and the fee paid....at this point quite frankly I was in autopilot and just wanted to get the hell out of there.

My wife was first presented with and asked to complete that affidavit in 2022, and again in 2023.

Last year it was presented in a new format from a template on their PC, with all the details included, just required signing.
